1/27/2024 EnchantaVaughn Approved QMHPA arrived to the home of Ms. Cosby. QMHPA informed Ms. Cosby that this was going to be an individual. QMHPA discussed with Ms. Cosby that this session was going to focus on increasing her social skills to assist her with reducing her depressive symptoms. Ms. Cosby did not present any symptoms of S/I or H/I at the time of the session. QMHPA began session by exploring with Ms. Cosby her current emotional well-being. QMHPA reviewed interventions from the previous session. QMHPA explored with Ms. Cosby her social skills and her interactions with her family members and people in the community that leads to depression. QMHPA encouraged Ms. Cosby to refrain from physical and verbal aggression and engaged in discussion with Ms. Cosby about the possibility of having an additional family member in the home. QMHPA reviewed with Ms. Cosby positive social skills and encouraged Ms. Cosby to continue engaging in positive activities in the community and to resume different coping skills such as walking to help her manage her physical health along with interact with others. QMHPA ended session by reviewing interventions with Ms. Cosby and encouraging her to provide feedback on the session. Ms. Cosby actively participated throughout the session. While exploring with Ms. Cosby her current emotional state, Ms. Cosby shared that she was in a positive mood. Ms. Cosby shared that her sister has not been in the home for the last two days therefore, she felt relaxed due to having the home to herself. Ms. Cosby shared that she continues to interact with others in her family, but that she has focused more on her own needs. Ms. Cosby shared that she has not engaged in any physical or verbal aggression, although she has continued to have verbal altercations with her oldest sister. While reviewing positive social skills to assist with decreasing depressive symptoms, Ms. Cosby shared that she will start walking once the weathers cools down to assist with her being depressed.
